Community-based services
At Cohese Healthcare (formerly GP Care), we work in partnership with Integrated Care Boards (ICBs), NHS Trusts, and other healthcare providers to deliver high-quality, community-based services to NHS patients. Our services currently include Community Urology and Deep Vein Thrombosis one stop diagnosis and treatment outpatient clinics. We provide care in the right place, at the right time—closer to home and tailored to patient needs.
To meet the growing demands of patients and communities, and to meet the objectives set out in the 10-year health plan, we are playing our part helping the NHS evolve with a more collaborative approach.
Our unique model
At Cohese Healthcare our model demonstrates how partnerships between the NHS, social enterprises, and private providers can deliver excellent outcomes and reduce pressure on secondary care whilst delivering services in accessible, community-based locations.
We offer a one-stop model that enables rapid mobilisation of services in areas of greatest need. Our model avoids unnecessary secondary care input by focusing on least restrictive interventions.
When the NHS works in collaboration with social enterprises and private businesses, it can better address patient needs and delivers excellent outcomes.

Our NHS Services
We continue to deliver community-based services to NHS patients free at the point of delivery. Our services are highly commended nationally and by those who use them, including community-based one-stop clinics in Urology and Deep Vein Thrombosis. These are just the type of solutions envisaged in the new NHS 10 Year Health Plan, offering accessible and integrated care.
